Home from Hospital

You are home from the hospital and have been getting home health physical therapy.  But, you have a caregiver who can’t be there when your therapist is there for training.  Your caregiver comes in the evening after work and wants training on how to help you get to the kitchen table for dinner, how to get you ready for bed, and how to help you get into bed.  They have notes from the therapist, but have questions and want to make sure they are doing it right.

You have been a caregiver for a couple years now.  But your mom just had a setback.  She ended up in the hospital with a new diagnosis, weakness, and requires more help from you for transfers. You have to get extra equipment to help her, like a wheelchair and a hospital bed.  How should you set it all up?  How do you use the new equipment safely?  How do you keep her safe?  How do you keep yourself safe and protect your body while doing more lifting with transfers and moving?  How much should you let her continue to do on her own?  What’s the best way to help her get dressed, use the bathroom, bathe, eat? I have worked as a physical therapist for the past 19 years in inpatient rehab and home health. I work with people with a variety of diagnoses and needs. I noticed many people and caregivers have a lot of questions when they return home from the hospital. I’d like to be an online resource for education and support to help patients and caregivers. I can help in many different ways: make your home safer, transfer training, equipment needs, how to
correctly do or progress exercises, bladder / incontinence management, understanding discharge instructions, referrals to other Healthcare professionals. I can make returning home easier and safer for you and your family.

1:1 Video/Phone Consultation

 

  • We provide 60 min Home Safety and Modification Assessments (video only) and Safe Transfer and Care Technique Training (video only) for personalized and expert guidance to make your home safer and your daily routines easier.  We will help you identify hazards, recommend changes or modifications to the home, and help with problem solving.  We help caregivers review transfers, assist with walking, and equipment use to safely protect yourself and your loved one.
  • We offer 30 min 1:1 General Question and Mobility Check-In (video or phone call) and Caregiver Support (video or phone call) options.  This is a shorter, focused session for quick questions, mobility tips, or caregiver guidance. Perfect for a progress check, troubleshooting a new challenge, or getting reassurance that you’re on the right track.
  • Schedule a time for your consultation, from the calendar, that is convenient to you for a 1:1 personalized consultation with a physical therapist.  Access our Calendly page via the buttons on this website. 
  • Pay through PayPal online with PayPal, Venmo, credit card, debit card.
  • Fill out the intake form and include your question/reason for your call, diagnosis, and any precautions or instructions from your doctor.
  • You will be sent a link for the meeting with instructions.
  • Connect with the therapist via Google Meet or Zoom (your choice) at the scheduled time.
  • Use the video call feature to show the therapist your home, exercises,
    transfers, etc for a more personalized session.

  • Questions will be answered via email, allow 24 hours for response.
  • Please include your question/reason, diagnosis, and any
    precautions or instructions from your doctor in your first email. You may send pictures to help the therapist understand and give more complete answers.
  • Expect some back-and-forth so that we can give you the best and most personalized information.
